It seems to have something to do with the distance at which you target the mob, the further away the more likely to be bugged. Also, it appears that you do no melee damage while target bugged (no hit/miss messages, can't hold agro). It happens to me most frequently when /assist'ing a puller on his way in to camp, or used to happen anyway, as I now tab to myself and back to a mob before engaging out of habit.
