Sarnak Emblazened Tabard
I remember snagging mine off a Sarnak within the Lake of Ill Omen, but it does have other drop locations.

It is also said to have dropped off various Kly in the Crypt of Dalnir. In terms of how rare that sucker was... I did a LOT of hunting in the Sarnak fort and I only saw one. It was your typical rare item off a subset of npcs within a zone.
