Did she have braces when she was younger? That could have caused staining that can't be whitened. If you can notice, I'm pretty sure she knows it also and has tried to fix the issue.
As in most relationships, you want to try and do it as a couple. You notice she is gaining weight, so you talk to her and say " Hey, I feel like I've been gaining a little weight. I want to start running/Join a Gym ect, and I want you to get in on this with me, so we can spend time together."
I don't know how transparent " Hey, these were buy one get one free. Would you like these? I don't think I'll need both boxes to lighten up my teeth.".
I think either way you're fucked, but good luck. Women are smart animals.
