There was no formalised universal PnP. True. However, GMs and Guides had the power to informally enforce sociable play. Without the regulation of the formal pnp the staff actually had greater unrestricted power to enforce desirable behaviour as they saw fit.

This is true. I was CSR on live in the classic era and it was basically server by server. There was a private site for people to submit their CSR shift logs and it had discussion forums where people would share how they'd been handling certain disputes on their server.
But it was totally dependent on who covered your server. Some GMs had different opinions on the degree of punishment for something, or whether there should even be a punishment for certain things.
