Correct. Backstab works like a normal attack, with five differences:
1. You have to manually press backstab.
2. Backstab adjusts your weapon damage before it does the damage calculation. A 15 damage weapon basically becomes a 120 damage weapon at 235 backstab skill. The formula for this is (Weapon Damage + 3) * ((Backstab Skill * 0.02) + 2). (15 + 3) * ((235 * 0.02) + 2) = 120. (120 weapon damage * 2) * 2.85 percentage multiplier cap + 11 damage bonus = 695 damage at best. The 696 we saw above is probably due to rounding.
3. A minimum damage is applied at 51+. For 51-59 it is (Level * 3 / 2). At level 60 it is Level * 2 = 120, which is the minimum damage we see on backstabs for level 60s.
4. The cooldown is not tied to weapon delay.
5. Double attack isn't unlocked until 55+.

553 / 1106 (duelist) is max backstab damage on p99.
While you're waiting to see an 1106 for yourself you can easily check 553's in your logs and confirm only higher backstabs were preceded by a duelist. The duelist message is "Soandso's eyes gleam with energy."
