It’s borderline dishonest, but I understand it

When you work in behavioral health, and I’m assuming the same is true for medical, you are taught that your documentation needs to support the outcome. This can become a very grey area, because while you cannot lie, people can choose which details to include or not include to paint a particular medical picture. One that supports your ultimate decisions

And this area can get pretty grey to various degrees based on the personal ethics of each clinician
