There's no using "one nuke", or stopping at either 3 or 5 targets. If you use a nuke, you know you're getting glassed as a second strike to prevent you from using the rest of your arsenal, so the initial play needs to be to eliminate your enemy. Which you can't do because in the time it takes for your ICBMs to arrive, they've already launched in retaliation.

China has those new hyper-sonic missiles everyone's worried about because it significantly reduces your decision-making time in that scenario, but even if you targeted all the nuclear launch sites you think you know about, you still haven't eliminated nuclear submarine second-strike capability.

They've war-gamed all this shit out since the Cold War, and that's why Mutual Assured Destruction is the best deterrent against anyone using 1+ nukes. First guy to launch starts a snowball effect into the effective destruction of all of human civilization.
