Confidentiality only exists where it is enforced. For a doctor, counselor, or lawyer to break it would mean that the state’s medical board or the state bar in the case of lawyers which could pull the person’s license to practice AND fine them

Other than that, confidentiality doesn’t really legally exist. It’s all a matter of how much do you trust the person
