Try setting your resolutions to be all the same in EQClient.ini.
this is mine for example..

FullscreenRefreshRate=0

HeightWindowed=1440
I don't use WinEQ2 and I don't inject any weird compatibility mode into the eqgame.exe either, these only will interfere with your diagnosing of the actual issue.
I use to have a lot of the issues you speak up, fixing it all took a while can't really remember everything I did but mine runs flawless now, but ya.. it took some messing around.
Also add this at the start of your EQClient.ini, allow multi-threading etc..

Maybe post your entire EQClient.ini
