The toughest games I was able to beat personally were "Super Ghouls N Ghosts" and "Battletoads in Battlemaniacs," both for the SNES. There are tougher games out there (some rather crazy japanese scroll-shooter games come to mind) but I haven't played them personally. I'm not sure how to rank some Atari games or old arcade-style video games because many of them did not have conventional endings and could not be beaten in any usual sense of the word.

MMO-flightsims like Air Warrior and Warbirds were also quite difficult to consistently succeed in, difficult enough the genre withered and died once easier genres came along. Online role-playing games tend to test other characteristics, such as knowledge or endurance.
