We found rivervale in our dnd campaign recently. Pretty sure the twist is that a bunch of different realities collided to create the DM's homebrew world, because the other areas we've seen were very specifically not EQ inspired. Seems rivervale, runnyeye, HK, and at least part of the karanas are mostly in tact, and the DM
did actually add bascially exactly what you're describing here. Pickclaws Vs Runnyeyes in the underdark basically.
