You are exempted by IP, not by account. So if your IP exemption is for 2 accounts, presumably you can have any 2 accounts running from the same IP, but not a third.

Not true, the exemptions are on the account not your IP.
Otherwise everytime your ISP decided to issue you a new Ip address (which they theoretically could do hourly if they wanted) you'd have to apply for a new exemption.
Instead, accounts with IP exemptions don't get counted as your 1 connection /ip. This means if you decided to create a separate account to level a cleric on (to share with friends/guild makes, for example) you'd need to request a new exemption. Although it's possible that you could still play it as long as the other account you were playing with had their own exemptions. But if you were both to create brand new accounts, you'd definitely need to apply for new exemptions.
