This is so not complicated and over-asked. It’s simple, once you go past server select you are logged in- those are the clients (per server) that they are counting. If that count is more than 1, you are boxing.

If you accidentally (whatever that would mean here) have a second toon at character select and you’re not doing stupid shit like quick logging for an unfair advantage, I don’t think they’re going to take serious/permanent action. If you do it all the time, or are obviously gaining an unfair advantage I bet you’ll hear from server staff sooner or later, and then we’ll see you on here whining about how unfair it is. And we will laugh.
