I like major things like, first to max factions (Either max bottom or max top), first to kill a difficult or well known NPC, but I think the first tier of enemy should be Crush/Dvinn level, stuff lower than that feels like it'll just be spammy (for a short while) and not terrifically interesting or really an "achievement" as the name implies.
The message will only go off once, for the first person to do it. Even adding this for tons and tons of mobs, I don't think it will get spammy. You'll get a lot of the lower end mobs out of the way within the first couple of hours on the server, and it'll be nice as everyone is rushing to the newbie yards searching for mobs to see a broadcast about a group of people able to take down Fippy [You must be logged in to view images. Although the guard getting the achievement would be pretty funny.
Not with my system :P

The end of quests seems good also, but for instance the monk sashes I think only the last two should be logged.
Congratulate all of them. Who cares.

"Being the first class" meaning like first class made?
No, the first of each class to certain levels. First Necromancer to 10 is a separate achievement and broadcast from the first Paladin to 10.

Would it be possible to be first to find specific areas of a given zone, noted for their secrecy or difficulty to get to? Take a kunark example of Kurns secret room. Perhaps a hail-able NPC could serve as the achievement granter there that despawns after the first person finds it, to solve the "Not tracking players" thing.
No, we don't log this.
