That is part of what WoW and some other MMORPGs went about solving as it was seen as a problem. The time, organization and know-how was just too involved. The key here is involved. The raids in WoW might be involved and I suspect that is why the level up is short and was looked over in later expansions. Same thing happened to EQ, 1-20 zone where everyone spawns and finish in 4 hours.
Cut out all of the player cities, forget its a RPG and focus on flavorless 'video game' that makes it more simple and easy.
Imagine being a Human Rogue player in Qeynos or Freeport trying to find their guild on release day of EQ.

Yeah I agree with blizzard with all of their complaints about classic everquest, and by far most of their solutions are great.
That said, Instances are stupid and terrible. Running one 50 times in a row, is not superior to killing something that re-spawns 50 times in a row, and its the exact same, I mean THE exact same, every time, while its a challenge to maintain a camp for 50 spawns and with other players entering and exiting the area, a wildly different and magical experience.
So they failed on fixing that issue.
