I know Saturn/Pluto in the 12th sometimes is about hidden enemies. It is the house of secrets, visions, and spirituality generally. It might be about a kind of spiritual blockage or intensity getting in the way of surrendering. It would also indicate that you have spiritual work to do on yourself over your life -- and that it is difficult/intense.
