I also was having big frame drops after recently restarting after a long break on a new 4 core win 10 laptop. It appeared the hard pauses coincided with players entering the zone (a constant issue in someplace busy like EC). Possible delay loading character info?
Reading various sources I tried several tweaks to resolve it as it made playing a bard honestly impossible.
A) editing config.ini with 2 lines to use only 1 core (don't have the laptop right now but it was 2 short lines),
B) setting everquest.exe to run in Windows 98 compatibility mode
C) druxa all in one over the install and had it add the additional launch config for multi core systems
~although right now launching p1999 from the recommended .bat provided in the v47 update seems more stable with less crashing when camping out.~
D) moved the EQ titanium install and backups off the hard disk drive to a solid state drive where the OS win 10 is installed on a separate folder.
E) changed in graphics config to cap frame rate at 60.
Don't notice dropped frames now but still some wicked screen tearing even at low resolution (default 4k resulted in a microscopic unplayable UI)
