Worst thing I did was actually back in 1999 within a few months of starting EQ itself.

I convinced my friend to play about the same week as I got the game, and we leveled/played together. One day, he got incredibly lucky and someone who was killing dwarf guards in Butcher Block ended up getting a Box of Abu-Kar, and they already had one. The guy shouted FFA for it and my friend got lucky. We were level appropriate for the zone at the time, so probably 10ish.

Some months later, after they removed the Box of Abu-Kar from loot tables, he was over at my house playing EQ with me (we were just barely teenagers at the time). I noticed he had his login details in a text file. I made note of them. I later logged into his account and traded his Box of Abu-Kar to me. I think I justified it to myself by feeling he didn't "earn" it, and that I played more than he did, or some other bullshit to make me feel okay about it in the moment. To attempt to make it seem like a bug, I even went as far as to buy a "Box of Lute Strings" from a vendor, which had the same graphic icon, and put it in its place. I even remember where I did it, in North Freeport near the bank.

When he logged on next and freaked out, I tried to reinforce the "it must have been a bug" angle, and he never ended up doing anything about it. In hindsight, I am not sure why he didn't decide to petition it -- believing it to be a bug or not. Maybe he knew/figured it out. I would've been outed by the GM and likely suspended/banned.

We continued to play EQ together for quite some time. He ended up getting bored of the game sometime just before Velious came out.

Of all the toxic garbage things I've done over the years on EQ Live, that stands to be the one that I actually regret and feel terrible over.
