Clear out your logs in your EQ\Logs directory - I do this by moving all log files to a new subdirectory, e.g. EQ\Logs\Archived. Run chkdsk /f and sfc /scannow in command prompt on your hard drives. If the issue still persists, do a new install of Titanium.
