I've crashed quite a few times for no particular reason. But I noticed that it ONLY happens when in the middle of soloing a mob and not in a group.

I think this may be the key, it's only ever happened to me whilst kiting. In all the hours I've spent running from place to place, grouping and raiding I've never known it to happen. Basically it happens at the worst possible time
