I noticed this on my necro once but the pet was failing to DW when given 2 of the SAME weapon. I gave the pet 2 minotaur battle axes in lower guk and he wouldn't dual wield. Strange. I must have been around lvl 39 when this happened. Maybe the "pet is carrying weapons" check just fails sometimes for other reasons?
For Frootloop, necro pets start to dual wield when given weapons at lvl 24( I'm 90% sure of this) and will DW without weapons at 44(100% sure).

See I wasn't 100% on the dual wield levels but I was under the impression that it was 20. Thought the pets followed the same line of warrior skills with double attack and dual wield at 15/20.
If what you say is true then pets are dual wielding earlier than they are supposed to. I did successfully test the lvl 20 pet with this identical weapon scheme. Can't foresee any other way that my pet is triple attacking on a regular bases if he isn't dual wielding.

Those "Quads" you talk about earlier is most likely 2x double attack.
