In west karana, there is also a spot where I fell through the world. This happened a month ago, but it was along the north side of the tunnel to qeynos hills. In the image below, approximately at loc (-150, -300) where the north wall comes to corner that points southward. You have to walk up and over the side of the wall with levitate. Unfortunately, it's pretty easy to accidentally do running out from qeynos hills with levitate on.
