99.9% of the time that's a resolution issue, mess with the default resolution in your game files

just edit X to desired resolutions, Just make sure it is supported by your monitor

Once you're in game, set it to 1080p or whatever your correct reso is.
Can also right click EQ on desktop>Properties>Compatibility>check Run in 640x480, this should let you in game where you can then change it in game.
