2. He is hitting at the high end of his spectrum WAY too much. Either his ATK is ridiculous (which I couldn't debuff because of issue #1) or AC is just not factoring in, because tanks were seeing max a LOT.
Against a level 60 Warrior (Defensive) his max hit is 437 (600 undefensive). This is the tanking summary for a tank during his entire defensive life.
Tanking summary for: Kinsen --- Total damage: 156523 ---
Avg hit: 414 --- Swings: 594 --- Defended: 89 (15%) --- Hit: 378 (63.6%) --- Missed: 127 (21.4%) --- Accuracy: 74.9% --- Dodged: 21 (4%) --- Parried: 38 (6.4%) --- Blocked: 0 (0%) --- Riposted: 29 (5.2%) --- Absorbed: 1 (0.2%)
He is averaging 414, with a max of 437. This is somewhat skewed via a minute or so of undisciplined tanking, but he is still hitting at the max end of the spectrum far too often.
A breakdown of the hits. As we all know, mobs have a range of 20 values which they can hit a player for. NEARLY 40% of these were max hits, over 50% were hits in the top 5 values:

39% of all connecting hits were MAX hits
54% were the 1-5th hits
17% were the 6-10th hits
16% were the 11-15th hits
13% were the 16-20th hits
Granted, this is without Velious raid gear, but the spread doesn't seem as even as it should be for a well geared warrior. I'm not as concerned with Tormax, as he is relatively easy in the grand scheme of things. But mobs with much higher max hits (Vyemm/AoW) will be hitting with this same spread of hits, likely even more skewed towards the high end of the range.
