He popped twice and then stopped popping with Guardian still up.
First kill he was wielding a single blade, 3 pieces of loot. Second spawn he didn't wield any blades at all and only dropped 2 pieces of loot. A ceremonial belt (currently dropping off KoS as well) and the common drop mage bracer. It seems like his loot table is off as well as his respawn?
