Bump. I just started and this is happening on my Monk.
It seems to happen when I try to throw right when I come into range with a target, I hit the hot key, and nothing happens. I wait for the cooldown, I hit throw again, then I get the error: Time not up. Attack -1, ranged xxxx no matter how close/far I am.
You have to wait 2 cool downs when this happens and make sure you are well within range before trying the third time.
