It was on all servers. Part of the reason Sony pulled it is because of the PVP element it brought to PVE.

I think you are right about that. But it was on the Test Server the longest. Maybe 3 weeks?? It was fun as hell though, but probably scared the crap out of a lot of noobs and they left for good lol. Not what Sony wanted on their Bottom Line though.
