I remember getting to Keeper when playing on the Grand Creation server. I had to be carried by 2 65's with full AA's to make it there, and it was still rough. I would imagine it takes at least a full group of solid 60's to make it on P99 without the shortcut.
Also, for anyone interested in the Paladin epic factions, Priests of Life faction is easily maxed by handing 2 gold to Lashun Novashine in Qeynos, although it takes a little while. For Deepwater Knights, the easiest way is to camp a mob in Toxxulia Forest named Elial Brook, a Level 7 Necromancer who spawns at 161, 1164. He has a pet with him and casts weak dots, but he should be pretty easy if you're at least level 10. Killing him earns Deepwater Knight faction, and it took me a few hours to max it off of him (he is on a 6 minute respawn timer).
If you don't want to become KoS to the Freeport Militia guards in Freeport, DO NOT ATTEMPT TO KILL SIR LUCAN OR BE PART OF A GROUP THAT KILLS HIM. He incurs a MASSIVE faction hit to Freeport Militia that will take you to scowling immediately, and will require orc scalps or goblin ears in Highpass to fix.
Finally, one of the most awesome things about the Fiery Avenger/Fiery Defender, besides the fact that it's a flaming 2h sword, is the fact that it effectively doubles as a shield, noted by FD's AC boost. What this means is that it is one of the very few 2h weapons in the game that you can Bash with, without the need for a shield. FA and FD both have this, the ability to Bash without a shield, which is awesome. The only other weapons I know of that can do this are the SK Epic (Innoruuk's Curse) and...POSSIBLY Nature's Defender?
Does anyone know the respawn timer on Lich of Miragul? Getting a team together for him when he's up will be the most difficult part of the Fiery Avenger, I'm thinking.
