You will not get the you are no longer hidden msg without getting hit by an npc who sees you. Being seen will not cause it to drop.
Something else is breaking it if this is happening to you. Perhaps attempting to open a trade or attempting to loot a corpse. Review logs or post them, perhaps this will help clarify your issue. Also there should be no way ghouls are seeing you, ghouls cannot see through IVU and most recent change is supposed to treat hide as IVU+Invis.
