Your plan, at the very least, would consist of:
1) Creating a new nation

This is something I agree with. The current system is set up to support certain elements of mainstream society that I believe have vested interests in the existence of unhealthy consumers. A new society
would need to be set up, and some people might think that is an unreachable goal; I consider it possible, even probable given the right circumstances.

2) Rounding up all the "healthy, educated" people of the world and placing them within your new nation.
- What do you do when a healthy person in your nation becomes unhealthy/fat? What do you do when an elderly person becomes senile? What do you do when someone loses their job/home and becomes malnourished due to lack of food? What do you do when an "unfit" child is born in your nation? Kick them all out? Euthanize them?

None of the above. Subsequent generations have been proven to be able to better respond to harmful stimuli.. If you want evidence, look at the numbers for smoking between the 30-50 age group and the 15-30 age group. Given the right society, where so much value isn't placed into advertising things that aren't healthy and a set of values to more thoroughly endorse education and health in society - contrasted to the current values we have which endorse convenience, comfort, and hedonism (mostly centered on eating patterns), I believe people would become more educated and healthy. I believe in this form of a society, people would use more energy towards goals which were positive for the society than they currently do through donations of time or money to (mostly) private charities etc..
This is really common sense stuff here. We've been poisoned by generations of research into how to shape our decisions. People in this country beat the drum of freedom until it is worn and weary, but seem to eternally gloss over this fact. We are only as free as our choices, and honestly the choice to live in a healthy way in city environments is not new, is was supplanted by less healthy massive private interests for a large period of time in the recent past.
Control the food supply, make certain things unavailable and promote certain things, and you would simply breed healthy, educated people over time, without the need to resort to any of these drastic measures:
