Needs to be customized to survive. EQ just isn't good PvP. Increase experience dramatically, something like x15 than normal or even more.
Bounty system, player killer who starts the fight becomes loot able coin and 1 item. items in bags are loot able too. 24hrs logged in before status goes back to normal granted no more starting fights the refreshes time.
Everyone starts with a call for help horn that allows you once per 24hrs logged in to summon a friend to your location.
Teams based on good and evil races would spice things up. Custom AAs which are PvP related for each class.
