EQ2 was the best game to utilize this, starting with Echos of Faydark [the best EQ2 expac when Scott Hartman took over or w/e his name was]. They had instanced raids, but they also had world raid mob versions of the bosses with better loot, and then completely random variance spawn "Avatars" that had the best loot in the game. Was a shit ton of fun, you had the static spawns you would pvp over, variance that tested bat phone, and still a ton of instanced content to satiate you. Unfortunately, the next expansion sucked and they never revamped combat like they promised to, but I feel they had the way content should be done right with that expac.
