The version we use on here right now was implemented on Live pretty early on during the Cleansing the Karana GM events. I
think that was pre-Kunark.
edit: looked on Allakhazam, earliest post I could find about that GM event was in 2001 and they said a level 54 received one of the following:

So I'm not really sure w/o wasting more time. Keep in mind, that item is soo good you'd never unequip it so maybe he wasn't 54 when it was received. I also heard about a level 6 druid getting this bp.
Just to make you drool, that effect is 14hp/14mana regen.
