I'm actually quite impressed how much support you play. Item builds are a little different than I'm used to but nothing too bad. I assume you're warding in the correct places and such?
One thing that revolutionized my support play is I actually took a lesson from someone. He gave me a couple pieces of awesome advice and it takes a while to get used to.
Stop what you are doing and ask yourself why you're doing it. "Why am I facechecking this bush with no one close to me." "Why am I warding the river brush when it doesn't give you vision on the gankpath"
In your nonranked games before you do something ask yourself why you're doing something before you do it. If you can't come up with a good reason don't do it.
Ward placement is really important, you can add free ganks on their jungler if you ward in the correct spots (a LOT of people ward in incorrect spots and just waste wards). Wards seriously win games.
When you go into a team battle you must have a plan before you do it, especially if you're the tank and/or if you have CC. STOP their damage dealers from killing yours, communicate with your team on who to kill first, and since you're support/tank a lot make sure you ping attack target on their main damage dealers that tend to go straight for yours.
I dunno if any if this will help you, you'll actually get worse for doing it at the beginning but in the long run it'll help you and you'll see the mistakes you've been making before you make them.
