If you press ALT + O you can open the game options. If you then click on the "Keys" tab you can see all of the key bindings. If you then pick the "UI" category, you can see all of the key bindings to show/hide parts of the UI. There are bindings for every different window/panel/etc. in the UI ... except for the EQ button.
AFAIK there is no way to show/hide that button. There might be some secret option in eqclient.ini, but I'm guessing you just moved it off-screen, either manually/accidentally or due to some bug. Either way, you can almost certainly fix it by doing ALT + O again, and clicking on the "Load UI Skin" button at the bottom of the "General" tab. If you load (or reload) a skin it should reset your UI, and move the button back on-screen.

You don't need the button for that. Each of the hotkey bars has their own key binding which can be used to show/hide them. You can show or change the hotkeys on that same "Keys" tab in the options, and then you can use those keys to show/hide the bars.

Move it off-screen (or at least all off-screen except for one pixel). Or move it behind something else.

I think that comes from the skin you choose. There are lots of different skins to choose from from that era (plus I think some P99ers have made their own), and I would imagine there are some with horizontal and some with vertical bars.
