Trapped under the ice
 

My Druid was walking across the frozen river, just east the temple with Barbarian Shaman vendor, and somehow dropped through the ice sheet. Once under, I could not resurface. Also, since I had a mammoth charmed, once the charm ended he able to hit me through the ice. I swam down and he followed. He killed me in the water.

Damn that sucks. Classic.

But damn that makes me laugh. There is indeed a hole in the ice that facilitates access to the miraculous tunnels. There is no bug you just were unlucky enough to stumble upon that hole in the ice. Something most people struggle to FIND.
