Area of effect spells

How did old-school classic/kunark AoE Groups work (pulling upwards of 50 mobs simultaneous) if AoE spells are capped at 4 targets?

Targeted AoE != PBAoE

Targeted = 4 mobs

Point Blank = unlimited mobs
