I second this thread resurrection.

As I was reading the first few posts, I thought "14 years later... the FPS change is the thing".

I don't know how my install got out of whack with respect to mouse look... but... I popped open the options and was trying for max fps. Dialed it to 60 and didn't even need to relog. It just worked.

Lookin' juss fahn witat mouse nayuh!
