Yeah that would be kinda like UO right? I hear lots of folks loved that game. The hardcore Pvp type players I have met over the past 18 years. Granted it's a subset of folks who like PvP but at least as many people as the max population logged into red99 at any given time.

There are a couple reasons why UO and EQ are barely even the same genre in this regard.
1. UO character progression based on skill based leveling vs EQ character progression based entirely on items.
2. A freshly created character in UO can actually damage a "max" level character
3. Items are a relatively minor component of character power vs EQ where it is almost entirely based on items.
4. In UO items are very easily replaced, cheap player made gear is like 80% of the effectiveness of the "top loot". Most everyone just played in high end smithed gear.
Item loot without level range would work terribly here. You seem overly concerned with stopping low to mid level griefing (which this would make 10000x worse btw). People need to learn to just power through it and move on, you can literally outlevel a level 20 PK in a couple hours.
I mean I could take a level 53 completely naked wizard and 1 shot anyone sub 35 or so with one lure. No RNG, no risk, just click a button and they die.
