Pretty easy to get to. Join a guild going into Fear. If they don't have any shamans that need the tear, (and no greedy people that would rather sell the MQ than help you,) then it's pretty easy to tag along and loot it.

Fyi I've seen on project 1999 servers you don't need max faction with true spirit.
I've seen someone turn in the tear for the final fight and turn in for his epic just after hitting warmly.
Supposedly there's no way to track the faction exactly, only what the mob cons, very unclassic.
