Ragefire Loot Table

Killed ragefire the other day and the only thing he dropped was the Heart for the epic. I feel like I've heard of this happening before, but can't remember if it is a bug. Should he always drop something off his loot table, or is there a legitimate slim chance to get nothing? Either way, glad to have my epic, thanks.

Response from guide, after confirming, is that 0 items is possible, just rare. Of course heart always drops.
