Wurmslayer should be changed to Primary only.

Unsure on the exact date when it was altered from Primary/Secondary:

10/04/2000

There are some items that cannot be equipped in the off-hand, like the
Wurmslayer or the Bladed Thulian Claws.

There are other items that are marked lore, so you can't have more than one
of them in your inventory.

Other than that, there are no restrictions on what can be dual-weilded.

You cannot have more than one item which is LORE. Some items also have hand
restrictions. Ebon Razor's are primary hand only, as are Wurmslayers...

Others are not lore and have no restrictions...so you can duel wield with
them if you want. ie: Yaks, langseax, etc.

Also no more ranged slot. Sorry folks.

Will continue working on pvp server until Epics are released.

It is currently 0.0 cast time. What needs to happen with this, and when, according to timeline?

There's patch notes where they did this to the staff of temperate flux, star of eyes etc because you could crash people's game.

This effect should remain instant until PoP era

just curious, if you have this item in secondary or ranged when it is patched, will it cause you to crash when trying to log in, will it auto-move to your inventory/cursor, or will you get to keep it in that slot until you take it out/die?

When Helm of Rile was changed from all large races to just iksar, it stayed in slot just fine until removed or death.
