This didn't happen to me, but I sat around waiting for the fight long enough that I asked questions, and here is what I think I know....
a monk went to turn in the Eye to an astral projection in LOIO to spawn the final fight (vorash/deep, xenovorash). He had turned in trunt's head in OT to get the eye. After turning in the eye in LOIO, nothing happened.
I know when I did this part of the epic for my monk, it worked just fine. Is it possible that someone handed the astral projection in LOIO some random item and that messed up the turn in? It just seems especially strange to me because it is a single item turn in.
Also, I saw in my chat windows the turn in monk Hail the npc and get the usually "Astral Projection ignores you" text (or something along those lines).
I'm mostly posting this to see if anyone can come up with a logic reason why this might have been bugged, and if not, then I'd hope the devs can fix it somehow. The monk turning the eye in was/is level 50+ and had done the turn-ins for gwan and trunt.
