I've done lord Rak'Ashiir for the shaman epic a couple times recently (3x), and I've never seen him use his aoe (just epic version, after the tear is turned in). The normal version uses it currently as far as I know, and the epic version should too. Here's a link to the aoe.

Now it's possible that we just killed him too fast to aoe. But I really doubt it, because I've seen it several times when we kill the regular version. Maybe someone else could confirm?

I've heard he should also gate, which I've never seen him try to do. A guy named Wagiet about halfway down the page talks about both abilities.

Rak lands has some nasty AE fire DDs (2K+ damage--1K in two ticks), rains, and DOTs. Rak WILL summon.

RAK GATES! When Rak is down to about 20% of health, he WILL GATE back to his original spawn spot. Be prepared for this event. If he gates, he will call in re-inforcements, so if it occurs, be prepared for multiple adds, from golems, ghosts, and Black Reavers.

Lord Rak`Ashiir can spawn from the corpse of a Black Reaver. Very important: he is KoS to anyone without sufficient True Spirit faction (possibly max ally). He is initially non-aggro until someone turns in the tear, someone attacks him, or someone with insufficient faction is nearby. Bring a raid, as he summons, gates, and has a nasty AE DoT as well. Once you give him the tear, he respawns as the epic version, and will try to kill the shaman who did the turn-in. Only the epic version drops the scale you need.

Once you give him the tear, he attacks immediately.

Also, when doing monk epic people mentioned that the astral projection, deep, vorash, and xenevorash should all spawn on the platform in the lake in loio. It is visible from the spot where they currently spawn. Right now the projection and all NPC's spawn on the shore in the SE corner of the lake. Here's a quote from 2003 talking about it. Turn-in eye to Astral Projection on platform, Deep and Vorash spawn and immediately agro. Pull to beach, had druid kite Vorash while we killed Deep. Then dropped Vorash and Xenevorash spawns on the platform. Everyone was pretty much still FM after those two so we pulled Xenevorash and quickly dropped him too.

Now head to the Lake of Ill Omen for the final fight of the monk epic. You can find Astral Projection (same form as the Overthere version) on the platform in the lake itself at +765, +860.

When you give the Eye of Kaiaren to the Astral Projection, he despawns. Deep and Vorash will then both spawn, and are very much KOS.

Deep procs Fist of Water, and hits for 170ish. Vorash procs Fist of Mastery, which is a DD and a stun, and hits for around the same as Deep.

Both Deep and Vorash have no items.

Once killed, Vorash says 'Foolish mortal! You think you have defeated me? Now, witness the true power of Rallos Zek!

Xenevorash will then spawn on the platform, regardless of where Vorash dies.

Upon further searching I found that the devs are spawning monk epic npc's on the shore because of an issue with the platform. Disregard the second half of the bug report please.

In regards to the fire dot, seems to be the RNG acting up. It's the same NPC after you handin the child's tear. The fire dot was setup as a proc however, which seemed silly, so it'll be a casted spell using its normal recast timer, pending update.

As far as gate goes, it is on his spell table, and it casts when it's supposed to as the code is written. But if that quote is correct about Rak gating as early as 20%, then the code for when NPC gate spells are able to be cast needs to be re-examined.
