Constant crashes after last patch, nothing else has changed.

Nothing about my install has changed, I have all the recommended fixes done and have for months, but after the last patch I'm crashing constantly.

Crashed again, was able to log in long enough to gate out of ToV and crashed again. Now I can't get in at all, I log in, go to the server select screen, pick my character, the loading bar completes and it crashes.
