Meldrath the Malignant should be level 24 (lower level) and his guards should be 37-38, from launch until October 1999.
It seems to have been changed in this patch (October 13th 1999):

For my gnome brothers and sisters this may be of some interest. Meldrath the Malignant, terror of the Steamfont mino caves, has become a formidable necro. Before the last patch he was roughly lvl 20 with two guards lvl 30 to 37. When I went to kill him yesterday with a ranger friend( apparently he drops a skull for ranger armor quests now) the ranger was scouting and told me Meldrath conned blue. I laughed since the ranger was 44 and I have killed meldrath multiple times on earlier occasions when meldrath conned green and I was about level 30. Anyway, Meldrath now conned white and his pet blue. He put up quite a fihgt, piercing for 50+ and casting SoP, fear and (for some reason) engulfing darkness like nobody's business. The fight was WAY harder before the patch since the pair of mino guards used to hit for about 88 with high magic resists. Now they hit for about 30 and my 34 pet ate one alive. Guess Verant didn't want to make life too difficult for those poor rangers...

Well you heard wrong. Meldrath was bumped up 10lvls plus of exp, he
use to be lvl 24, an his body guards where lvl 37-38. Now it is
reverse it seems. The body guards are prob 28plus but below 31. and
mell is as stated 35-37.
