I had this same issue and installed WinEQ2, but the problem is it is always in windowed mode that fills the screen even with a fresh .ini file.

Fullscreen just doesn't work well with this game. The quicker you come to accept that, the faster you will experience relief. I now play in windowed mode in 4:3 ratio as the game runs fantastic this way as it was meant to run that mode.
It sucks...I want full screen on my widescreen monitor too. But this game is old.

P99: Dont go overclock your CPU, Don't go fullscreen fixes 60% of issues. Dealing with antivirus fixes another 30% of issues. Rest is people not knowing about updating files and 1% actual issues.

I had this same issue. I first had to change my resolution to a 16 by 9 ratio (1600x900) and then I hit the full screen button.
Nevermind it seems to be an issue with green and not blue.

I resolved this with EQ2 + duxaui. I ran Duxa's All In One Installer tool once to get the duxaui (still have to /load duxaui once in game) and then ran the Duxa All In One Installer tool a second time to change to fullscreen mode. I set the resolution to match my native windows resolution of 1920 x 1080
