The Rivervale piranhas drop the wrong teeth.
Rather than dropping the generic stackable tooth, they should drop their own unstackable type of tooth with a different icon.
Likewise, the various quests (http://everquest.allakhazam.com/db/quest.html?quest=51) in rivervale requiring piranha teeth should only accept the tooth that drops in Rivervale (http://everquest.allakhazam.com/db/item.html?item=49606).
You'll note that the Allakhazam actually lists the wrong tooth that is dropped by the piranha/used in the quest, but if you use magelo to check the actual tooth dropped by the piranha (https://eq.magelo.com/npc/48585) you will see it is the one I linked (and not the generic piranha tooth available elsewhere).
Edit: the allakhazam entry also includes the faction hits of each step.
Jump in to the lake and start killing piranhas (be careful as they tend to come in pairs - and keep an eye on your air meter). Kill the trout to get large piranha to spawn, be careful as Chomper may also spawn!
Upon turn-in of 1 piranha tooth at least dubious faction, Fiddy Bobick says, 'Whew! We are sure on the way to saving this village, pal! They're gonna erect a statue in our names. Fishslayers is what we are! Let's keep up the good work!'
