So far this has only happened for me in upper guk.
Other group members have verified it is happening to them too.
When someone pulls 2 mobs and I use /assist and then auto attack:
-My character does not attack. (no animation or damage)
-My character does no melee damage.
-The mobs HP does not reflect the HP% decrease as group members attack it.
To fix it, I have to hit "ESC" and re-target the mob.

I have a similar problem in SoldungB fighting Sonic Bats. Occasionally while meleeing a sonic bat (fighting at the bat zoneline to SoldungA) I'll stop swinging at a bat, with no error messages, even though autoattack isn't turned off and I'm not out of range (ripostes still land).
One of the things that seem exacerbate the issue is if I have a /target hotkey bound to my movement keys (in this specific instance /target Guano). Each time this happened, although the guano wasn't up to take target and my target window still claimed I was targeting the original sonic bat I'd stop making any attacks against the bat at all (again excepting ripostes).
Similarly I have to clear target and reselect.

Someone reported this to me yesterday (after the patch) and fixed it by removing that characters UI files.
Check and see if it happens to you on other characters

I will test that out the next opportunity I get. This bug was happening for me pre and post patch.
