I could be wrong, but I don't think anything is supposed to get 5 attacks per round in Velious. It happens fairly frequently, too, and even after I filter out ripostes.

60+ mobs can triple attack on main hand swing on this server

has been that way ever since they were fucking with vyemm early on in velious

Seems like a bug then? http://everquest.allakhazam.com/db/npc.html?id=5509&p=3#comments has many references to quadding, none to 5 per round.

This is an intentional "non-classic" difficulty change as far as far as I am aware.
